Signing the Data Connection Agreement

In order to use your network port you must fill out and sign the Data Connection Agreement with the University of Idaho. You can get a copy of this agreement online at the URL http://www.uidaho.edu/csrv/res_policy.html. Print out a copy from Netscape in the student computer labs on campus.

This agreement asks for several pieces of information so the University can keep records and activate the correct locations port. Most items are self-explanatory with the exception of Faceplate Number and Ethernet Card Hardware Address.

Faceplate Number is code for which building on which floor, in which room, and which port you're going to plug your card into. Find the salmon-colored phone-like jacks in your room, and read off the numbers on the sticker on top of the jacks. Each jack has a letter associated with it, A, B, or C. A is usually the jack you plug your phone into, and B and C are both the data jacks. Pick B or C and that should be the last letter in the code. Here is an example of a code for jack B:

039 C 209 - 01 B

The Ethernet Card Hardware Address can be found using a diagnostic utility that comes with Windows 95.

  1. Click the Start button in Windows 95.
  2. Select Run from the menu.
  3. Type msd in the run field and hit Enter. The utility will start at this time.
  4. Hit the N key for network. This will display the network card information.
  5. Copy the Physical Address field onto the form.

Once you have read over the agreement, sign it and bring it to the second floor of the Wallace Center for Leo Stevens. It will take about a week to process, and you will be emailed or phoned when your connection is active.
